- MyHealth Online: If you are a current Granite Medical patient, you can use the MyHealth Online patient portal to make appointments, request prescription refills, or request referrals. If you are a current patient who has not signed up, please enroll in MyHealth Online here.
- Records Release (pdf): Download this form if you want your medical records forwarded to a third party. Due to privacy laws, this form cannot be submitted online. Patients must print this document, sign it and either fax or mail it to Granite Medical Group.
